PATIENT CARE COORDINATOR - 2 OPENINGS
Education: A.A. in Nursing and/or BSN
Experience: 1 year of experience of providing home health/hospice nursing care to patient. Previous supervisory experience also preferred. A knowledge and understanding of medicare.
Duties of position: Provide leadership and day-to-day supervision of field staff who provide deliver of care to home health patients/clients. Assure compliance with agency standards and state/federal laws and regulations.
Qualifications: Licence RN in State of Minnesota. Provide EXCELLENT HIGH QUALITY CUSTOMER SERVICE.
